Root cause
A high, narrow palate is the floor of the nose and the ceiling of the tongue. When that space is small, nasal airflow is tighter, the tongue sits low, and sleep can stay fragmented even after a full night in bed. Masks and mouthpieces can manage the symptom. This consult is about whether the structure itself can be widened.
The protocol
This page is only about bone-anchored adult expansion finished with clear aligners at the same time. It is not a removable appliance and it is not a generic lab expander. Cases in the DOMEzero protocol are coordinated with the DOME planning team. The point of the “zero” plan is skeletal widening without the large space between the front teeth that traditional expanders leave.

No. Classic DOME is a surgically assisted expansion associated with Dr. Stanley Liu’s work. DOMEzero is a bone-anchored protocol coordinated with clear aligners and a no-diastema plan. The consult is where we say which one, if either, fits your sutures and your airway.
The protocol on this page is planned to avoid the large diastema people associate with older expanders. Aligners run at the same time as expansion. Small transitional spaces can still happen. A guaranteed invisible course is not something any honest office can promise.
No. Widening the maxilla can increase nasal volume and, in selected adults, improve sleep-disordered breathing. It is not a cure, and it does not replace a physician’s diagnosis. Some patients still need medical therapy after expansion.
Expansion alone is $4,500 to $6,500. Concurrent aligners are $5,500. A complete breathing plan often also includes myofunctional therapy, a tie release if the exam shows one, and QuietNite soft-tissue treatment. Added together, that complete plan is about $16,000. You are not billed for pieces you do not need.
